At How to write a scholarly research report you'll find some useful information on how you might organise your final research report.  Bearing in mind that you may not be going to publish your research (although the opportunity will certainly be provided through the ASLA(NSW) newsletter) you probably won't need to go into as much detail as is suggested here.

In an action research report it would also be useful to include a concluding section that discusses the implications of your research for future practice.
